Do you have a Medical Assistance or Medicaid plan with a Physical Health HealthChoices Managed Care Organization?

If so, your plan might no longer be available in Jefferson County as of September 1. You will need to choose a new plan by August 16 or one will be automatically chosen for you.

Though additional plans show they are available for Jefferson County on the Pennsylvania Enrollment Services website, as of September 1 Punxsutawney Area Hospital will only accept the following plans:

·UPMC for You
·AmeriHealth Caritas
·Geisinger Health Plan (excluding Punxsutawney Physician Group providers)
